Back-to-School FormsNEW + RETURNING STUDENTS: read and complete the Beginning of Year documents and have your student return completed forms (*) to their grade level student orientation. Students entering or advancing to 7th grade must also provide evidence that they have received a pertussis-containing vaccine (e.g. Tdap) on or after their 7th birthday and a 2nd dose of Varicella.
If your student is unable to make orientation, please return required forms via email to Ms. Rachel at [email protected]. Ms. Rachel can also be reached in-person at the Science Academy main office if you have questions regarding these forms or the enrollment process in general. |
